Three suspects in custody after Colorado officer killed responding to call

A Colorado police officer was killed while responding to a call about a suspicious person, Fox News confirms.
Lakewood Police initially reported that the suspect in the shooting was also killed, but have since changed that statement and are only reporting that the officer is killed, FOX31.com reports.
Officers responding to the call at 2 a.m. Friday heard shots being fired in the neighborhood located near Denver. They were able to remove three residents from the house the shots were coming from before more were fired.
Police have not identified the suspect or the officer killed, but said he was with the department for six or seven years and was in his mid 30s

Report: Lack of police training factor in shooting

LAKEWOOD-A panel of law enforcement experts found several weaknesses that led up to the fatal shooting of Agent James Davies last fall.
Davies was accidentally shot by a fellow police officer during a shots-fired call last November. Agent DJ Braley thought Davies was the suspect behind a fence.

One of the many recommendations included in the report are as follows:

Recommendation:
Establish a system of a documented After-Action Reviews or Report (AAR or structured debrief) to be disseminated throughout the chain of command and the training unit. The participants and organizers of special enforcement actions or events complete AAR's. AAR's are used to determine what happened, why it happened and how it can be done better in the future.
