NEW HAVEN, Conn. -- The New Haven Police Department released on Wednesday its report into the crash that killed Officer Dan Picagli.
Picagli died days after the Oct. 17 accident in which he was hit by a sport utility vehicle while he worked overtime at a construction site.
The four-month investigation found a lack of accountability for the crash, saying work-zone signs were missing and construction cones were not put out properly. Investigators said the construction company had an expired work permit at the time of the accident.
Recently released transcripts of 911 calls the night Picagli was struck reveal the beginning of an ordeal that resulted in the death of husband and father of three children.
The first caller said, "He was hit … and he's lying on the ground."
The second caller elaborated on the accident, saying, "He's hurt, he's badly hurt … it's badge number 300."
The driver of the SUV, President George W. Bush's second cousin, U.S. Judge John Walker, will not face charges.
Investigators concluded in their report that Walker was driving between 23 and 29 mph when he struck Picagli, who ran New Haven's police athletic league
In the wake of Picagli's death, the New Haven Police Department has committed to more training for officers who work with road crews. City officials will inspect work zones for safety regulation compliance before road work begins at any site.
