That kind of "reporting" here in Mass. might find one standing before a judge....

Sergeant, I did not watch the video. Judging just by the transcripts, I can see that some of the officer's responses seem downright strange, but why the empasis on the complaint form? I don't see how a complaint form is important at all. If you have a complaint about a police officer, and you want to put it in writing, put it in writing and send it to the police department addressed to Rat-, uh, I mean Internal Affairs or to the Chief of Police and it will get attention. What I see here is an anti-police group trying to make a story where there is none.

It's been my experience that if a person walks into a police station, they want to talk to someone, whether it's to report their dog missing, report a crime... or file a complaint about a cop. If someone walked into your department and said he wanted to report a crime what would you do? If he asked for a form to report the crime would you give him one, or would you ask questions to try to figure out what the issue is? My guess would be the latter.

From the transcript I see some behavior I don't agree with, but for the most part, I see departments saying "talk to a supervisor" or "let me take a report"...

I'm not sure that there exists an easy answer to any of these situations. We don't have any forms and would only ask someone to "reduce it to writing" after asking some basic questions much like many of the Florida officers did. From watching the video, it might not have been the message that went afoul but maybe the manner and methodology employed by the officer.
I believe that if an agency is looking in Mass. to get accreditated they have to show compliance with a four or five step process that's loaded with forms for "citizen complaints".
